A page with no main heading
A screen reader user asks "what is this page?" and gets no answer — there's no top-level heading naming it. On a product page that means they can't confirm they're looking at the right item before adding it to the cart.

Steps for your store platform
WooCommerce
Most themes make the product or page title the H1 automatically. If yours doesn't, edit the page and set the top title block to Heading 1.
Squarespace
Open the page → add or select the text block with the page title → set it to Heading 1 in the text toolbar. Every page needs exactly one.
BigCommerce
Storefront → Pages → make sure the page title is present, or ask your theme developer to output the page title as an H1.
